These are the list of documents required for a new property purchase or for property refinance:

For Purchases:

  1. Two (2) most recent pay check stubs
  2. W-2 for the last two years
  3. If self-employed need tax returns for last two years (all pages) and profit and loss statement for the current year
  4. Last three month bank statements for both, checking and saving accounts (copies of all pages)
  5. Most recent statement of 401-k, 403B, IRA or other retirement plans
  6. Copy of sales contract including all addendum
  7. Copy of Earnest Money Deposit Check (front and Back)
  8. Listing and selling agent telephone and fax numbers
  9. Closing attorney or title company who will handle the closing for both parties (buyers and sellers)
  10. Copy of lease agreement on rental properties (if applicable)
  11. $350.00 check for appraisal fee payable to Nationwide Financial Services Group, Inc.

For Refinance:

  1. Two (2) most recent pay check stubs
  2. W-2 for the last two years
  3. If self-employed need tax returns for last two years (all pages) and profit and loss statement for the current year
  4. Last three month bank statements for both, checking and saving accounts (copies of all pages)
  5. Most recent statement of 401-k, 403B, IRA or other retirement plans
  6. Last Mortgage statement received from mortgage company
  7. Declaration Page of Homeowner insurance and agent information
  8. Deed
  9. Title insurance
  10. Survey
  11. $350.00 check for appraisal fee payable to Nationwide Financial Services Group, Inc.
